If you are interested in using or renting our facilities, please complete this rental request form and turn it into our church office. The church staff will then determine the availability of the requested facility and rental date and contact you to further discuss facility rules/guidelines as well as rental fees. 

If you are part of one of the sports teams using our facilities, we must have one of these sports release forms signed and on file PRIOR to you being aloud to use our gym facilities - once signed, you can send it into our church office.
