If you are interested in signing up for one of our Summer Camps (VBS, Safety Town, or Sewing Camp), please complete this Registration form and email it to Ruthy@syvpc.org 

If you are interested in using or renting our facilities, please complete this rental request form and turn it into our church office. The church staff will then determine the availability of the requested facility and rental date and contact you to further discuss facility rules/guidelines as well as rental fees

Rich discipleship is a key component in the mission of SYVPC. To advance our discipleship in 2024, we want to learn about our Small Group participation. Please help us do that by taking just a few minuets and completing this short survey.

If you are part of one of the sports teams using our facilities, we must have one of these sports release forms signed and on file PRIOR to you being aloud to use our gym facilities - once signed, you can send it into our church office.

If you would like to become a member of our Church, or if you are a current member but some of your information has changed, please fill out one of these Membership Information Forms and return it to the Church office.
